mapping is not showing

Issue summary: 

I have been using this for about 5 years.  I set up a map for my credit card. 

The map is no longer showing.

 

When I import the converted file into my accounts, my accounts will not accept it saying there is a problem.

prevous mapping shows up as x-d out in the tab mappings. not able to use them
