Issue summary: 

I have tried to convert a CSV file that I could previously do and that I have not been able to do for three months ... I have tried to do it since the previous version LEGACY 4 and this if it does, then I do not understand what is going on

I converted your CSV with no problems, edit this issue to see the attachments.

I mapped it as
